I knew what I wanted for my career before I attended the
Center of Excellence-Harvard Business School Value-Based Healthcare Course, but
I did not have the vocabulary to name it or the understanding of payment models
for it be financially viable. I am interested in a career centered on orbital
and neuro-ophthalmologic imaging and dream of a reading room integrated into an
integrated practice unit with regular imaging rounds and interdisciplinary
discussions.

As a neuroradiology fellow, I appreciate the value of being
a trusted consultant for clinicians. The crown jewel of being a radiologist is
working directly with a clinician to incorporate a patient’s history and
physical to their imaging to generate a meaningful differential diagnosis for
treatment planning. Naturally, an interdisciplinary conference is the most
efficient way to provide the highest quality of care. These conferences
required dedicated time—at times a lot of it—to thoughtfully discuss each
patient. While radiologists immensely enjoy the collaboration, the sense of
untended clinical duties and patient studies piling at the work station can
distract from the meaningful work at hand.
While the current model rewards producing a greater number
of products (or imaging reports), Value-Based Healthcare and time-driven
activity-based costing provides a model to reward radiologists for the value of
time and expertise devoted to each patient’s diagnosis and management. In light
of this new model, art and science of imaging consultation in integrative
practice becomes as financially sustainable as it is clinically necessary. In
this way, value-based healthcare recognizes the highest level of care as the
basis of care and can allow radiology to be truly part of integrative and
valuable care.
Moving forward, I can present these ideas and analyses to a
future employer to empower them to allow their faculty to share our greatest
gifts.
